Abstract

Background: Social isolation is a common mental health issue among patients with schizophrenia. This condition is characterized by a decline in the ability to interact and a tendency to withdraw from social environments. If not properly addressed, this condition can hinder the recovery process and reduce the patient’s quality of life. Objective: To describe the application of nursing care for patient Mrs. H with the nursing issue of social isolation in the Bangkoang Ward at Duren Sawit Regional General Hospital. Method: A descriptive case study of one patient using a nursing process approach, ranging from assessment, nursing diagnosis, intervention, implementation, to evaluation. Data collection was conducted through interviews, observations, and review of medical documentation. Interventions were provided over 3 days using a phased implementation strategy that included exercises in self-introduction, conversation, and participation in group activities. Success was evaluated using the SOAP approach based on changes in the patient’s social interaction abilities. Results: After the intervention, the patient demonstrated improved social interaction skills, marked by the ability to introduce oneself, engage in simple communication, participate in group activities, and increased eye contact. Withdrawal behavior also appeared to decrease gradually. Conclusion: The implementation of nursing care through specific strategies has a positive impact on the social interaction skills of patients experiencing social isolation. Recommendation: These strategies can be implemented as a mental health nursing intervention to help improve the socialization skills of patients with schizophrenia.
