Abstract

INTRODUCTION: Coronary Heart Disease (CHD) remains a serious threat to the operational readiness of soldiers, an irony considering this highly-fit group is generally presumed to be low-risk. This reality strongly suggests that conventional lipid screening often fails to detect hidden, underlying CHD risk. The main objective of this study was to thoroughly examine and record the detailed lipid profile patterns in 96 male Indonesian Army soldiers newly diagnosed with CHD, in order to identify the most dominant dyslipidemia phenotype among them. METHODS: We adopted a retrospective descriptive study approach, analyzing the medical records of 96 male Indonesian Army soldiers with confirmed CHD managed at RSPAD Gatot Soebroto during 2024. The data focused on laboratory results for Total Cholesterol, Triglycerides, LDL-C, and HDL-C. This data was then processed using simple descriptive statistics to determine the magnitude and distribution of these lipid abnormalities. RESULTS: Although the majority of patients were recorded as having Total Cholesterol and Triglycerides within the normal range, our key finding was the high proportion of patients with very low High-Density Lipoprotein (HDL) levels, coupled with unfavorable LDL fractions. This pattern clearly concludes that atherogenic dyslipidemia (a lipid abnormality that drives the hardening of blood vessels) is the primary type of lipid disorder in this soldier cohort. CONCLUSION: It is concluded that cardiovascular risk in these active soldiers is not primarily driven by outwardly obvious high blood fat levels, but rather by hidden disturbances in lipid fractions. We recommend a greater focus on routine screening for low HDL and Non-HDL-C, alongside the implementation of more specific lipid therapy strategies to protect the long-term health of military personnel.
