Abstract
Schizophrenia is a chronic mental disorder with a high relapse rate, strongly influenced by patients’ adherence to treatment. Non-adherence is often the main cause of symptom deterioration, and although pharmacological therapy cannot fully cure schizophrenia, regular medication intake has been proven to reduce symptom severity. This study aimed to examine the association between medication adherence and symptom improvement in outpatients with schizophrenia at Dr. Mintohardjo Naval Hospital during May 2025. Using an observational analytic cross-sectional design and total sampling technique, 11 patients who met the inclusion criteria were enrolled. Data were collected from initial and final PANSS scores and medication adherence was assessed using the MMAS-8 questionnaire. Statistical analysis included Shapiro-Wilk, homogeneity testing, One-Way ANOVA, and Post Hoc Tukey HSD. The results showed that most patients (54.5%) demonstrated high adherence with significant reductions in PANSS scores, where ANOVA revealed p=0.010 and an average reduction of 24.00. Further analysis with Tukey HSD confirmed a significant difference between high- and low-adherence groups (p=0.008). In conclusion, medication adherence was significantly associated with symptom improvement, with highly adherent patients experiencing greater improvements compared to those with moderate or low adherence.
