Abstract
Background: Osteoarthritis (OA) is a degenerative joint disease commonly found in the elderly, particularly affecting the knee joint (genu) due to its weight-bearing role. This condition frequently leads to complaints such as pain, morning stiffness, limited range of motion (ROM), and decreased muscle strength, which subsequently hinder daily functional activities. Objective: This study aims to determine the outcomes of physiotherapy interventions in reducing pain and increasing the range of motion in a case of Osteoarthritis Genu Dextra. Methods: This study utilized a case report design conducted on Mrs. N (66 years old) diagnosed with grade II OA genu dextra at Dr. Mintohardjo Naval Hospital, Central Jakarta. Interventions were administered over 5 sessions (April 6 – May 7) utilizing a combination of Ultrasound (US), Transcutaneous Electrical Nerve Stimulation (TENS), and exercise therapy consisting of Hold Relax and Closed Kinetic Chain Exercise (CKCE). Evaluation parameters included the Visual Analogue Scale (VAS) for pain, a goniometer for ROM, and the WOMAC questionnaire for functional ability. Results: After 5 therapy sessions, there was a decrease in tenderness from a score of 1.8/10 to 0.8/10 and motion pain from 5.3/10 to 1.4/10. The active flexion range of motion of the right knee increased by 15°, from (0°-0°-120°) to (0°-0°-135°). There was an increase in the muscle strength of the right knee flexors and extensors from 4/5 to 5/5. Additionally, an improvement in functional activity was observed, indicated by a decrease in the total WOMAC score from 46/96 (moderate category) to 25/96 (mild category). Conclusion: The consistent delivery of physiotherapy interventions, including Ultrasound, TENS, and exercise therapy, is proven effective in reducing pain, increasing range of motion, enhancing muscle strength, and improving functional capacity in patients with Osteoarthritis Genu Dextra.
