Abstract

Diabetes mellitus (DM) accompanied by dyslipidemia triggers oxidative stress that accelerates the pathogenesis of coronary artery atherosclerosis. Blueberry fruit (Vaccinium spp.) is rich in anthocyanins, flavonoids, and triterpenoids, which have potential as vasculoprotective antioxidants. This study aims to determine the effect of blueberry fruit extract (EBB) on the histopathological improvement of the coronary arteries in diabetic Sprague-Dawley rats. This post-test only control group design divided the rats into six groups: K1 (normal), K2 (high fat diet + alloxan), K3 (high fat diet + alloxan + simvastatin), and K4, K5, K6 (high fat diet + alloxan+ EBB at doses of 125, 250, 500 mg/kgBW) with a 14-day treatment period. Diabetes induction was performed using alloxan at a dose of 125 mg/kgBW. Parameters were evaluated through coronary artery histopathological scores using the Hematoxylin-Eosin staining technique to assess atherosclerosis progressivity. Kruskal-Wallis analysis showed a significant difference in histopathological scores between groups (p=0.002). The Mann-Whitney test indicated that the most optimal histopathological improvement of the coronary artery occurred at the dose of 250 mg/kgBW (K5), which closely approached the normal group (p=1.000), differed significantly compared to the diabetic group K2 (p=0.011), and demonstrated an improvement equivalent to simvastatin (p=1.000). This vasculoprotective effect confirms an hormesis dose-response relationship, where the lowest dose acts as the best therapeutic window before reaching a pro-oxidant phase at high doses.
