Abstract

Pneumonia is a lower respiratory tract disease that remains a serious public health problem in Indonesia and around the world. Pneumonia is an acute infection of the lower respiratory tract characterized by coughing and difficulty breathing, which arises from various infectious agents such as viruses, bacteria, mycoplasma, fungi, and foreign body aspiration (Syafriningrum & Sumarsono, 2023). Pneumonia patients who come to the emergency room generally show clinical signs of dyspnea, tachypnea, decreased oxygen saturation, and secretion accumulation. One of the measures that can be applied to help improve respiratory function is the use of the Active Cycle of Breathing Technique (ACBT). ACBT is a respiratory physiotherapy technique that aims to assist in the mobilization and removal of secretions through three stages, namely: breathing control, thoracic expansion exercise (deep breathing exercises to improve lung ventilation), and forced expiratory technique/huffing (forced exhalation to push secretions out) (Jones et al., 2025). This scientific paper aims to analyze the results of ACBT therapy intervention in pneumonia patients to reduce shortness of breath and assist in sputum removal in the ER. The method used was a case study. After two interventions per day, there was a decrease in respiratory rate and an increase in the ease of sputum clearance. ACBT can be applied as an evidence-based nursing intervention that supports medical management of pneumonia patients in the ED. This technique is simple, does not require special equipment, can be performed independently by patients with guidance from nurses, and has been proven to help improve oxygenation and airway clearance effectiveness.
