Abstract
ST-elevation myocardial infarction (STEMI) is a severe form of acute coronary syndrome associated with substantial morbidity and mortality, despite the widespread use of primary percutaneous coronary intervention (PPCI). Length of stay (LOS) represents an important clinical indicator reflecting treatment response, disease severity, and healthcare burden. Changes in cardiac and inflammatory biomarkers are presumed to influence hospitalization duration. This study aimed to analyze the association between changes in cardiac and inflammatory biomarkers and length of stay among STEMI patients undergoing PPCI. This analytical observational study employed a cross-sectional design using medical record data of STEMI patients treated with PPCI at RSUP Persahabatan, Jakarta, from June 2022 to June 2025. The analyzed variables included changes in high-sensitivity cardiac troponin I (hs-cTnI), red cell distribution width to platelet ratio (RDW/platelet), hemoglobin to RDW ratio (Hb/RDW), and triglyceride to high-density lipoprotein ratio (TG/HDL) in relation to length of stay. Data were analyzed using univariate, bivariate, and multivariate statistical methods according to data distribution. The results demonstrated that changes in selected cardiac and inflammatory biomarkers were significantly associated with length of stay in STEMI patients after PPCI. These findings suggest that a multimarker approach may provide additional prognostic value in predicting hospitalization duration. This study may contribute to risk stratification and optimization of clinical management in STEMI patients.
