Abstract

Erectile dysfunction (ED) is a common complication of diabetes, often linked to diabetic neuropathy and associated with declines quality of life (QOL) but is frequently overlooked due to limited awareness and social barriers. Despite the high prevalence of diabetic neuropathy in Indonesia (54%), research on diabetic neuropathy patients' QOL, particularly those experiencing ED, remains limited. This study aims to describe the characteristics of diabetic neuropathy patients with ED and determine the association between ED severity and QOL. This was a descriptive cross-sectional study. Data was collected with sociodemographic questionnaire, International Index of Erectile Function (IIEF‐5) and World Health Organization Quality of Life - BREF (WHOQOL-BREF). Descriptive and spearman's correlation analyses were employed for statistical analysis. The median age was 57 years old, 57,1% had diabetes for 1–5 years, 53,6% were classified as obesity class I, and 46,4% had mild-to-moderate ED. Quality of life was generally moderate in the physical domain and good in psychological, social, and environmental domains, while overall health was mostly poor (46,4%). Significant negative moderate correlations were found in the social relationships domain and overall health (p<0.05). No significant associations were found in the other domains (p>0.05). This study found that erectile dysfunction in diabetic neuropathy is associated with lower quality of life in social relationship and poorer overall health perception, emphasizing the importance of multidisciplinary approach in clinical and non-clinical management.
