Abstract

Non-communicable diseases (NCDs) such as hypertension and diabetes continue to rise each year. In response, the government and BPJS Kesehatan established the Chronic Disease Management Program (Prolanis) as a promotive and preventive effort. However, several challenges remain in implementing Prolanis at Pasar Minggu Health Center, including the dual roles of health workers, irregular participant attendance, and the low achievement of the Controlled Prolanis Participant Ratio (RPPT). This study aims to evaluate the implementation of Prolanis at the Pasar Minggu Health Center using a qualitative descriptive approach. Data were collected through in-depth interviews, observations, and document reviews. Results showed that Prolanis implementation is running well. Input aspects include the availability of health workers, funding, guidelines, and supporting infrastructure. Activity aspects such as health consultations, medication services, supporting examinations, and group activities are carried out regularly. Output aspects show improvement in RPPT achievement. Success is supported by the competence of health workers, interprofessional collaboration, and adequate facilities. However, obstacles remain, such as limited staff, digital system issues, and low participant engagement. To address these, strategies such as limiting daily participants, involving community cadres, staff training, and providing education and incentives for participants are applied to enhance Prolanis overall effectiveness.
