Abstract

Manufacturing industry is the second largest contributor of work accident cases in Indonesia. PT X, a metal pipe manufacturing industry company, has had 2 cases of work accidents in the welder work unit in 2024. Work accidents factors have to be identified so work accidents can be effectively managed and losses prevented. The purpose of this study is to analyze the causes of work accidents using the HFACS and ECFA methods in the welder work unit of PT X in 2024. This research is a qualitative research with a case study design. Data sources were taken through in-depth interviews with informants and company documents review. 4 informants consisting of HSE Supervisor, HSE Officer, and 2 accident victims were selected using purposive sampling method. The results obtained are the contribution of factors in all 4 layers of HFACS levels and 3 factors from ECFA. The generic causes of the accidents are improper work techniques, sub-optimal equipment, high workload, lack of training, and unspecific SOPs. Recommendations for the company are to improve work SOPs, conduct routine equipment maintenance, conduct training, and improve working hours management. Comprehensive efforts can fill the failure holes that cause work accidents in the welder work unit.
