Abstract
Emergency treatment for children in the emergency room often involves the administration of intravenous (IV) fluids to meet fluid requirements and facilitate medical therapy. This procedure frequently causes pain and anxiety, which can lead to uncooperative behavior in children. Therefore, a therapeutic approach is needed to divert the child’s attention, helping them remain calm and cooperative—one of which is distraction through watching cartoon videos during the IV insertion process. This study aims to evaluate the effect of distraction using cartoon videos on reducing pain and anxiety levels in children aged 6–12 years in the emergency department of Gatot Subroto Hospital. Children within this age range are generally capable of understanding and responding to interventions and assessment tools. The distraction intervention was applied during IV insertion, and evaluation was conducted using the Wong-Baker FACES Pain Rating Scale to assess pain levels and the Children's Fear Scale to measure anxiety. These tools allow children to select facial expressions that best represent their feelings, thereby increasing data validity. Measurements were taken before and five minutes after successful IV insertion. Pain and anxiety assessments were conducted by the children themselves, their parents, and independent observers. The results indicated that the distraction method was more effective in reducing anxiety than in alleviating pain. Pain reduction appears to require additional strategies to achieve optimal outcomes. Based on these findings, it is recommended that nurses consider using cartoon videos as a distraction technique during IV insertion procedures in children. Future research is also encouraged to include control groups, enabling more objective evaluation of the effectiveness of cartoon video distraction.
