Abstract
Medication error is one of the serious problems that can have a direct impact on patient safety. This study aims to evaluate the implementation of PKPO in efforts to prevent medication errors in the Pharmacy Installation of the Drug Addiction Hospital (RSKO) Jakarta in 2024. This study uses a case study approach with a qualitative method. Data obtained through interviews, observations, and document reviews. The evaluation was based on the implementation of 8 PKPO standards. The results of the study showed that the implementation of PKPO in the Pharmacy Installation of RSKO Jakarta had met all standards, but for the overall implementation of PKPO in preventing medication errors, it only reached a total of 93.25% of the target set, which was 100%. Supporting factors are supported by competent human resources, compliance in reporting incidents, adequate policies and budgets, and the implementation of an effective e-prescribing system. However, there are still obstacles in storage standards due to limited warehouse capacity and in prescribing standards related to shadowed e-prescribing writing. In addition, there is a lack of human resources, inaccuracies in the dispensing process, and infrastructure that is not yet fully in accordance with standards. It is necessary to conduct an evaluation of the number of human resources, the addition of storage warehouses, and supervision from the Ministry of Health regarding the implementation of existing standards in preventing medication errors in hospitals.
