Abstract
Congestive Heart Failure (CHF) is a chronic condition resulting from the inability of the heart to pump blood optimally, leading to decreased cardiac output and impaired blood distribution throughout the body’s tissues. Common symptoms include shortness of breath, prolonged fatigue, and edema in the extremities or lungs due to unmanaged fluid accumulation. This study aims to examine the implementation of both interventions in CHF patients with a nursing diagnosis of decreased cardiac output, while also identifying the clinical responses demonstrated by the patient throughout the care process. The study employed a case study approach based on nursing care conducted over three days on a CHF patient at RSUD Pasar Minggu. Data were collected through in-depth interviews, direct observation, periodic physical examinations, and medical record reviews as documentary sources. The interventions provided included consistently positioning the patient’s body at an angle of 30–45 degrees and playing Quranic murottal recordings adjusted to the patient’s tolerance, accompanied by daily evaluations to monitor condition progress. Over the three days of intervention, the patient demonstrated measurable improvement, marked by a gradual reduction in shortness of breath complaints, vital signs approaching normal ranges, increased comfort, and improved tolerance toward daily activities. The Semi Fowler position proved effective in expanding lung capacity, thereby reducing the heart’s workload, while murottal provided relaxation stimulation that decreased anxiety and enhanced the patient’s psychological calmness. The combination of both interventions is effective as a non-pharmacological nursing approach for CHF patients with decreased cardiac output and can be recommended within nursing care standards to support holistic recovery and comprehensively improve patient quality of life.
