Abstract
Fracture is a condition characterized by the disruption of bone continuity caused by trauma or excessive pressure that exceeds the structural strength of the bone. One of the common medical procedures performed to restore bone alignment is Open Reduction and Internal Fixation (ORIF). This procedure aims to reestablish bone stability; however, it often results in postoperative acute pain due to tissue trauma, inflammatory responses, and the insertion of metal implants. Poorly managed pain may hinder mobilization, delay the healing process, and negatively affect patient comfort and quality of life.One non-pharmacological intervention proven effective in reducing acute pain is the application of coldpack compress. This therapy works through a vasoconstriction mechanism that reduces local blood flow, inhibits inflammatory mediator release, decreases tissue metabolism, and slows the transmission of pain impulses to the central nervous system. In this practice, the coldpack compress was applied for 15 minutes to the postoperative clavicle fracture area. The implementation of Evidence-Based Nursing demonstrated a reduction in pain intensity from 7 (severe pain) to 5 (moderate pain) based on the Numeric Rating Scale (NRS). The patient appeared more relaxed, showed reduced facial grimacing, and was able to rest comfortably after the intervention. Based on these findings, the coldpack compress is proven effective as a complementary therapy for reducing acute pain in postoperative clavicle fracture patients. Furthermore, this study aims to analyze the nursing care process, beginning from assessment, diagnosis, planning, implementation, and evaluation in applying coldpack compress therapy. The results support recommending this intervention as part of Evidence-Based Nursing practices for postoperative pain management in fracture patients.
