Abstract
Patient safety is a crucial indicator of healthcare service quality, which must be maintained through systematic and continuous implementation of safety procedures. One of the key efforts to achieve this is the application of Patient Safety Goals (PSGs), aimed at preventing the occurrence of Patient Safety Incidents (PSIs) during the care process. This study aims to evaluate the implementation of PSGs in preventing PSIs at the inpatient wards of RSPAD Gatot Soebroto, a national referral hospital. The study utilized a qualitative approach with a descriptive design, employing in-depth interviews, direct observation, and document review. Informants included healthcare professionals such as nurses, on-duty physicians, and members of the hospital’s quality and patient safety team. The findings indicate that the implementation of PSGs has been relatively effective and aligns with hospital regulations, supported by the availability of Standard Operating Procedures (SOPs), adequate facilities, and a good level of understanding among health workers. However, challenges remain, particularly the limited number of human resources in several wards, which affects the continuity of PSG application. PSI data from 2024 to 2025 revealed that Potential Harm Conditions (KPC) were the most frequently reported incidents. This highlights the need for ongoing improvement to ensure optimal patient safety in all aspects of care delivery.
