Abstract
Background: Chronic Kidney Disease (CKD) is a global health problem that has a very high prevalence. Hypertension on the other hand is one of the main causes and complication that can worsen the patient’s condition. For CKD patients in the final stages, hemodialysis (HD) is the main therapy used. However, it is important to note that hemodialysis cannot improve kidney function, this process only serves to maintain the patient’s quality of life. Objective: To comprehensively describe nursing care for Ckd On Hd patients with hypertension at RSPPN Panglima Great General Soedirman. Carried out through a series of processes, this process includes assessment, determining nursing diagnoses, nursing interventions, nursing implementation and evaluation of nurisng that has been provided. Method: The research used in this research is qualitative with a case study approach. Data collection was carried out by interviews, observation, physical examination and documentation. Results: The assessment showed that the patient had several main nursing problems, including acute pain related to physical injurious agents, risk of ineffective cerebral perfusion related to hypertension, and disturbed sleep patterns related to environmental obstacles. Nursing interventions focus on pain management, vital sign monitoring, and sleep support Evaluation showed that the patient's condition improved after three days of intervention. This case study emphasizes the importance of a holistic approach. in nursing care for Ckd On Hd patients with hypertension to improve the patient’s quality of life. Conclusion: Nursing care given to Mrs. D can be resolved. It is hoped that future researchers can carry out nursing care for Ckd On Hd patients with hypertension properly and appropriately so that it is achieved in accordance with the nursing problems found in the patient.
