Abstract
Rupture perineal grade II is a mother which giving birth the baby at the first time with rupture perineal’s organ caused by natural lacerated, incision procedure, or both. The rupture will be creating severe pain. This observation wasconducted to examine the applicationof nursing care to Mrs. S with rupture perinealgrade II includes..nursing..assessment, nursing..diagnosis, nursing..intervention, nursing..implementation, and nursing..evaluation. Research Methods : a descriptive design was employed utilizing a case study approach. Research Results : Identified nursing issues in this case included acute pain associated with physical injury, limited mobility due to pain, and an increased risk of infection linked to invasive procedures. After administering nursing interventions for 72 hours by providing non-pharmacological anti-pain exercises, active range of motion (ROM) exercise, and vulva hygiene exercise for 20 minutes a day, the.results.of.the.nursing evaluation.were.partially.resolved with the objectives and outcome criteria set.Conclusion : Practice of nursing care to Ny. S with rupture perineal grade II reaching the wishes result because there is a good therapeutic relationship betweennurses and patients and other health teams
