Abstract
Introduction. Breast cancer is the most commonly diagnosed malignancy among women worldwide over the past five years, with Indonesia reporting the second-highest incidence in Southeast Asia. Family history, particularly mutations in the BRCA1 and BRCA2 genes, plays a critical role in increasing susceptibility and exacerbating the malignancy of breast cancer.. Obesity is a significant risk factor for breast cancer and contributes to its malignancy through mechanisms such as increased estrogen synthesis, chronic inflammation, and insulin resistance. The grade of malignancy is crucial for determining prognosis and selecting appropriate therapeutic strategies for breast cancer patients. Methods. This study employed an analytical observational design with a retrospective approach. Sampling was conducted using primary data from interviews and secondary data from patient medical records through consecutive sampling. From a total population of 168 patients, 74 were included as the sample. The frequency distribution of each variable was described in univariate analysis, while the relationship between variables was analyzed using bivariate analysis with the chi square test. Results. The analysis revealed a significant association between family history and the grade of breast cancer malignancy (p=0.008; OR=6.47) as well as between body mass index and the grade of breast cancer malignancy (p=0.030; OR =3.09). Discussion. These findings indicate that family history and obesity are significant risk factors contributing to higher grades of breast cancer malignancy. This underscores the importance of increasing awareness of these factors, as early detection and preventive strategies could reduce the prevalence and severity of breast cancer malignancy.
