Abstract
Background: Diabetes mellitus is a metabolic disorder or problem in the process of breaking down nutrients from food into energy in the body, characterized by chronically high blood sugar levels due to disruptions in insulin production or the body's response to insulin. Diabetes mellitus is classified into type I, type II, other types, and Gestational. This study was conducted to determine the appropriate interventions for type 2 diabetes mellitus patients. Research Methodology: This research employs a descriptive method with a case study design focusing on discharge planning, encompassing the stages of Assessment, Nursing Diagnosis, Interventions, Implementation, and Evaluation. Results: The main nursing problem identified from this study is blood glucose instability related to insulin resistance. The primary nursing intervention undertaken is monitoring blood glucose levels and educating about dietary compliance. Conclusion: The implementation of care for Mrs. S, who suffers from Diabetes Mellitus (DM), successfully achieved the desired goals due to the excellent collaboration among the nurse, the patient, and the patient's family. Suggestion: It is hoped that the patient can apply the education that has been given and the family can provide support to the patient.
