Abstract
In the large and fast development of the industry, both old and new companies need machines with advanced technology to meet market demands and improve processes. Production processes that use advanced technology can cause unavoidable side effects such as work accidents that can threaten the safety and health of workers. Recorded in the historical data in the K3 section of PT. XYZ since 2017 s.d. In 2018 there were 21 work accidents in the production environment. After observation, there were nineteen work accidents, 90% with the type of work accident in the form of a fall, one of 5% with the type of work accident in the form of falling goods, and 5% of work accidents with the type of work accident. chain-related accidents. To find out the hazard results, the HAZOP, FTA, and Fishbone Diagram methods are used to determine whether there is a risk of work accidents on workers as well as efforts to prevent, reduce and even eliminate the risk of work accidents in the workplace. PT. XYZ has 10 findings, then categorized according to the source of the danger into 5 sources of danger, namely working environment conditions, damaged reporters, leaking roofs, damaged glass supports, work materials. Recommendations for improvement are to provide training to workers so that workers increase knowledge so that accidents do not occur, by making software to detect work accidents.
