Abstract
Heart disease remains a significant cause of morbidity and mortality in Indonesia, with a prevalence of 877,000 cases. The high burden of this disease is linked to various modifiable factors, particularly lifestyle factors such as physical activity, fruit and vegetable consumption, and body mass index (BMI). This study aimed to analyze the associations between physical activity, fruit and vegetable consumption, and BMI with the incidence of heart disease among the adult population in Indonesia. The study utilized secondary data from the 2023 Indonesian Health Survey, employing a cross-sectional design. The sample consisted of 614,222 respondents aged 18 years and older with complete data, following a weighting process. Bivariate analysis was conducted using simple logistic regression. The results showed a heart disease prevalence of 1.0%. Vigorous physical activity (OR=0.334) and moderate physical activity (OR=0.523) were associated with lower odds of heart disease compared to the group engaging in no physical activity (p=0.000). Respondents with BMI categories of wasting (OR=0.369), normal (OR=0.471), and overweight (OR=0.793) had lower odds of heart disease compared to those with obesity (p=0.000). Conversely, fruit and vegetable consumption showed an association contrary to the study hypothesis (OR=1.503; p=0.000). These findings may be influenced by the limitations of the cross-sectional design, the possibility of reverse causality, and the presence of confounding factors that could not be fully controlled. Therefore, further research using longitudinal designs and multivariate analysis is needed to obtain stronger evidence regarding the relationship between lifestyle factors and heart disease incidence.
