Abstract

Surgical site infection (SSI) is a common complication in patients undergoing orthopedic surgery for open fractures and many increase morbidity as well as length of hospital stay. Appropriate and guideline-adherent use of prophylactic antibiotics is a crucial strategy for preventing SSI. This study aimed to evaluate the use of prophylactic antibiotics and its association with the incidence of SSI in patients undergoing open fracture orthopedic surgery at Bhayangkara Tk. I Pusdokkes Polri Hospital. This study employed an observational retrospective design using medical record data from inpatients treated at Bhayangkara Tk. I Pudokkes Polri Hospital in 2024. A total of 25 patient medical records were included using a total sampling technique. The evaluation focused on the appropriateness of antibiotic selection, timing of administration, and dosage based on established prophylactic antibiotic guidelines.The results showed that most patients received prophylactic antibiotics with appropriate timing and dosage according to the guidelines. The timing of prophylactic antibiotic administration was significantly associated with the incidence of SSI, whereas the appropriateness of antibiotic type and dosage showed no significant association. These findings indicate that the prevention of SSI in orthopedic open fracture surgery depends not only on appropriate antibiotic selection but also on the timely administration of antibiotics, dosage and other clinical factors.
