Abstract

Background: The advancement of technology has led many people to spend long hours using electronic devices in static sitting positions with insufficient physical activity, increasing pressure on the neck area and contributing to the development of Cervical Herniated Nucleus Pulposus (HNP) with complaints of pain and functional impairment. Physiotherapy serves as the primary solution in managing this condition, utilizing Transcutaneous Electrical Nerve Stimulation (TENS) and Ultrasound (US) for pain management, Myofascial Release (MFR) and Stretching Exercise to improve range of motion, Isometric Exercise to strengthen paracervical muscles, and Pressure Biofeedback Unit (PBU) to enhance deep neck flexor muscle strength. Objective: This study aimed to determine the effectiveness of physiotherapy management in Cervical HNP cases through four intervention sessions over two weeks. Methods: Assessment tools included the Visual Analogue Scale (VAS) for pain level, goniometer for range of motion, Manual Muscle Testing (MMT) for muscle strength, palpation for muscle spasm, Craniovertebral Angle (CVA) measurement for forward head posture, and the Neck Disability Index (NDI) for functional activity. Data were collected through anamnesis, measurements, and observation during therapy sessions. Results: After four intervention sessions, there was a reduction in pain, improvement in range of motion, increased muscle strength, decreased muscle spasm, and improved functional activity. Conclusion: Overall, this study demonstrated that the combination of TENS, US, MFR, Stretching Exercise, Isometric Neck Exercise, and PBU was effective in reducing pain, decreasing muscle spasm, improving range of motion, increasing muscle strength, and enhancing functional activity in Cervical HNP patients.
