Abstract
Thrombolysis and mechanical thrombectomy are established treatments for acute ischemic stroke. However, the role of the National Institutes of Health Stroke Scale (NIHSS) as an early surrogate marker of neurological improvement after combined therapy remains unclear. Objective : To evaluate changes in NIHSS scores before and after thrombolysis followed by mechanical thrombectomy, and to describe patient characteristics, risk factors, imaging findings, and treatment time metrics associated with these changes at RSUP Fatmawati. Method : A retrospective cohort study was conducted using total sampling of acute ischemic stroke patients treated with thrombolysis and thrombectomy from January 2023 to December 2025. Variables included demographics, clinical risk factors, baseline NIHSS, imaging findings (ASPECTS, occlusion site), treatment time metrics, and post-therapy NIHSS. Analysis was performed using the Wilcoxon Signed-Rank test. Result : Among 31 patients, 48.4% improved, 41.9% deteriorated, and 9.7% remained unchanged, with no significant difference in NIHSS scores (p > 0.05). Patients were predominantly aged 55–65 years (32.3%) and male (77.4%), with hypertension (64.5%) and dyslipidemia (71.0%) as common risk factors. Most had ASPECTS ≥6 (80.6%) and middle cerebral artery occlusion (64.5%). Efficient door-to-CT and door-to-needle times were achieved in 74.2%, while door-to-puncture time was >120 minutes in 90.3%. Baseline NIHSS was mainly moderate-to-severe, and post-therapy distribution showed increased severe cases but emergence of mild stroke (6.5%). Conclusion : No significant difference in NIHSS scores was observed; however, clinical improvement occurred in a subset of patients. Outcomes appear influenced by heterogeneous clinical, imaging, and treatment factors. Further studies with larger samples and multivariate analysis are needed.
