Abstract

Stroke is the second leading cause of death worldwide, with ischemic stroke being the most common type. Stroke is a condition that requires prompt and timely management. One of the therapeutic options is intravenous thrombolysis using recombinant tissue plasminogen activator (rtPA). However, the success of this therapy may be influenced by various factors, one of which is the patient’s blood glucose level. Several studies have shown that hyperglycemia in patients is associated with poorer clinical outcomes, including a reduced response to thrombolytic therapy. Nevertheless, to date, there are no clinical guidelines that establish elevated blood glucose levels as a contraindication to thrombolytic therapy in patients with ischemic stroke. This study aims to analyze the effect of random blood glucose (RBG) levels on thrombolysis outcomes in acute ischemic stroke patients at the National Brain Center Hospital Mahar Mardjono Jakarta. This study is an observational analytical study with a retrospective cohort design, using 96 secondary data from medical records. Samples that met the criteria were analyzed using Spearman correlation tests, as the data were measured on an ordinal scale. The correlation between RBG levels and changes in NIHSS scores in acute ischemic stroke patients undergoing thrombolysis showed a correlation coefficient (r) of -0.011 with a significance value (p-value) of 0.917. The relationship between RBG levels and changes in NIHSS scores is very weak and negative. Since p-value > 0.05, this relationship is not statistically significant. Therefore, it cannot be concluded that elevated RBG levels are an independent factor affecting changes in NIHSS scores in acute ischemic stroke patients.
