Abstract
Appendicitis is the most common cause of acute abdominal pain in children. This study examines the association of abdominal pain duration, leukocyte count, and body temperature with perforated appendicitis in children. A retrospective case–control study was conducted using medical records of children aged 3–18 years with intraoperatively confirmed appendicitis at An-Nisa Hospital, Tangerang, during 2023–2025. The case group consisted of perforated appendicitis and the control group of non-perforated appendicitis, with 20 subjects in each group. Bivariate analyses used Chi-square, followed by binary logistic regression. Most patients were aged 13–17 years and female (55%). Duration of abdominal pain ≥48 hours occurred in 67,5% of subjects and was more frequent among perforated cases (85% vs 50%; OR 5,67; 95% CI 1,25–25,61; p=0,043). Leukocyte count >12,000/mm³ was observed in 62,5% and was strongly associated with appendiceal perforation (90% vs 35%; OR 16,71; 95% CI 2,98–93,89; p=0,001). Temperature ≥38°C was not significant (p=0,107). In multivariate logistic regression adjusting for age and sex, only leukocyte count remained independently associated (adjusted OR 12,19; 95% CI 1,64–40,66; p=0,015). In conclusion, leukocyte count emerged as a significant risk factor for the incidence of perforated appendicitis in children, highlighting its value in early risk assessment and clinical decision making.
