Abstract

Objective: Rational antibiotic use was essential to maintain treatment effectiveness and prevent antimicrobial resistance. Pneumonia was one of the most common infections requiring antibiotics, and its increasing burden required structured evaluation of antibiotic utilization. This study aimed to evaluate the pattern of antibiotic use in pneumonia inpatients at Dr. Mintohardjo Naval General Hospital in 2023–2024 using ATC/DDD, DU 90%, and its categorization within the WHO 2025 AWaRe classification to support rational antibiotic management and reduce resistance risk. Method: This was a descriptive quantitative cross sectional study using retrospective medical record data. Antibiotic use was analyzed with ATC/DDD, DU 90%, and AWaRe classification. Result: Pneumonia inpatients were dominated by female patients and age group >65 years. A total of 21 antibiotics from 10 classes were used, with overall consumption of 104.27 DDD/100 patient days. Ceftriaxone injection 1 g/vial was the most frequently used antibiotic, contributing 39.84% of total use and reaching 32.93 DDD/100 patient days. DU 90% analysis identified six antibiotics, including Ceftriaxone, Levofloxacin, Meropenem, Cefoperazone, Azithromycin, and Ceftazidime. Based on AWaRe classification, 96.01% of antibiotics were classified as Watch. Conclusion: Antibiotic use in this study was dominated by broad spectrum Watch group agents, with Ceftriaxone as the most frequently used and forming the core of the DU 90% segment. The high reliance on these antibiotics highlights the need to strengthen antimicrobial stewardship and conduct ongoing qualitative evaluations to ensure rational and resistance aware prescribing.
