Abstract

Data information for each health service is required and collected through the Health Information System (HIS). One form of HIS implementation at Puskesmas is the Puskesmas Recording and Reporting System (SP3) which functions to support the process of monitoring, evaluation, and planning of health programs, including the Nutrition Program. However, there are problems, namely the lack of understanding of Puskesmas employees, inaccurate recording and negligence being late in collecting reports by the person in charge at the Puskesmas Kecamatan Pancoran. This study aims to analyze the implementation of the SP3 in the nutrition program at Puskesmas Kecamatan Pancoran in 2025 and was conducted for two months in May-June 2025. The informants were the person in charge of implementing SP3 in the Nutrition Program as well as the data and information center staff. This study used qualitative methods, with data collected through in-depth interviews, observation, and document review. The results showed that the implementation of SP3 in terms of input, process, and output has not gone well. In terms of inputs, Human resources are adequate, but equipment and SOP understanding remain unevenly distributed. In terms of process, recording and reporting have followed the specified flow, although there are still delays caused by the late Integrated Health Post (Posyandu) schedule, which has an impact on the delay in sending data to the Puskesmas Kelurahan and Puskesmas Pancoran. Timeliness of reporting is influenced by discipline, coordination, and cadre support. Strengthening coordination is needed, such as training, increasing the capacity of human resources, and regular reminders to maintain the quality and accuracy of reporting.
