Abstract
The referral system at primary healthcare facilities ensures patients receive appropriate care at higher-level health facilities. At Puskesmas Kembangan, implementing the referral system for JKN participants faces challenges such as limited coordination between referrers and recipients, insufficient resources, and lack of socialization. This study evaluates the referral system for type-2 DM patients under JKN at Puskesmas Kembangan in ensuring the quality of health services in 2024. Using qualitative methods, data were collected through in-depth interviews, observations, and document reviews. The results for input, process, and output variables align with existing standards and regulations. Supporting factors include drug availability and health facilities. While drug availability is not a primary reason for referrals due to distribution limitations at first-level health facilities based on the National Formulary, the availability of diagnostic facilities is the main cause of referrals. Inhibiting factors include limited human resources and insufficient patient knowledge about the referral process. Puskesmas Kembangan has strategies to address these issues, but output results highlight obstacles in reliability, indicating the need for continuous service improvements. This summary condenses the study findings while retaining the essence of its analysis and conclusions regarding the referral system's strengths, challenges, and areas for enhancement
