Abstract

Objective The National Health Insurance Programme has been running for more than a decade. The number of registered participants as of 31 December 2023 was recorded at 267,311,566 people. Although its implementation has been running for a long time, problems that arise in practice still continue to occur. The purpose of this study is to analyse the implementation of justice in accordance with the basic principles of bioethics and the potential for inequality in the Cileungsi Regional General Hospital. Method This research uses mixed methods, which is a combination of qualitative and quantitative methods. This quantitative method uses a univariate quantitative descriptive analysis study design to determine the perception of justice felt by national health insurance patients. The quantitative sample consisted of 96 samples using a random sampling method that met the exclusion and inclusion criteria. The qualitative sample consisted of key informants who knew or were involved in BPJS services. Result The results of the three-level categorisation show that the justice perceived by patients for each aspect of distributive, procedural, interactional and informational justice is at a moderate level. This is confirmed by the results of interviews that show positive perceptions from each interviewee. The test results of the Binomial Test results, on distributive justice p-value = 1.000 (>0.05), procedural justice pvalue = 0.475 (>0.05), interactional justice p-value = 1.000 (>0.05), and informational justice p-value = 0.184 (>0.05). This indicates that there is no statistical evidence to reject H0, so although most patients feel fair, this result is not statistically significant to state that the principle of fairness has been fully implemented. Conclusion Based on the results of the study, it was found that the level of justice at RSUD Cileungsi tends to be classified in the moderate category, both distributive, procedural, interactional, and informational justice. There is no significant difference and statistical evidence to state that the principles of justice have been fully implemented. However, the tendency of positive responses in each aspect of justice can illustrate that RSUD Cileungsi has implemented justice in accordance with bioethical principles.
