Abstract

Background: Coronary Artery Bypass Graft (CABG) is a crucial cardiac surgical procedure for treating coronary heart disease. This study aimed to describe the characteristics, clinical conditions, intraoperative procedures, mortality, morbidity, and length of hospital stay outcomes in patients undergoing CABG at the RSPAD hospital from 2020 to 2023. Methods: This study employed a descriptive method with a retrospective cohort approach. The sample consisted of 71 patients who underwent CABG at RSPAD from 2020 to 2023. Results: The majority of patients were male (93%), early elderly (46- 55 years old), with hypertension comorbidity (83%), normal to overweight nutritional status, and utilized the government health insurance (BPJS Kesehatan). During the intraoperative CABG procedure, most patients underwent the implantation of 3 grafts, with an average CPB time of 207.9 minutes, cardiopulmonary bypass (CPB) time of 112 minutes, and a circulating blood volume of 923 cc. CCT, CPB time, and blood transfusion volume were higher in the mortality group of patients compared to the surviving group. The postoperative mortality rate was 25%, with higher risks among female patients, elderly patients, those with diabetes mellitus (DM) and hypertension comorbidities, and those who received a single graft implantation. The leading causes of mortality were Low Cardiac Output Syndrome (44.5%), Shock (44.5%), and Multi-Organ Failure (11%). The incidence of post-CABG morbidity was 66%, with the most common complications being pneumonia (31%), VT/VF (25%), sepsis (15%), and AKI (14%). Higher risk was observed in male patients, those of early elderly or elderly age, patients with comorbid DM and hypertension, and those receiving 3-4 grafts. The majority of patients had a post-CABG hospital stay of 1 week, with a tendency for longer stays among female patients, those with DM comorbidity, elderly patients, and those receiving 3-4 grafts. Conclusion: The findings of this study indicate high rates of mortality and morbidity among postoperative CABG patients, with the main risk factors being female gender, advanced age, DM and hypertension comorbidities, and the number of grafts implanted. These findings can serve as a basis for improving the management and care of CABG patients in the future.
