Abstract

Primary health care facilities serve an indispensable role as the initial point of contact and gatekeeper within the healthcare system. Nevertheless, this function is frequently compromised by the persistence of the bypass phenomenon, wherein patients directly access secondary or tertiary health care facilities. This study aimed to investigate the determinants associated with patient preferences in selecting primary health care services at Clinic X, Bekasi City, in 2026, utilizing the Andersen Behavioral Model as the theoretical framework. An analytical-quantitative study with a cross-sectional design was conducted among 94 respondents (comprising adult patients and companions of pediatric patients), selected through an accidental sampling technique. Data were collected via structured questionnaires and analyzed using the Chi-Square test. The findings revealed a statistically significant association between cost affordability (p=0.000), telemedicine utilization (p=0.000), and perceived need (p=0.000) with the selection of primary health care services. In contrast, predisposing factors, geographical accessibility (distance and travel time), mode of transportation, and evaluated health status exhibited no significant correlation (p>0.05). In conclusion, the optimization of primary health care utilization necessitates strategic interventions, primarily through the integration of remote digital services, healthcare tariff adjustments, and sustainable public health education.
