Abstract

Pulmonary tuberculosis is an infectious disease caused by the bacterium Mycobacterium tuberculosis. This infection not only attacks the lungs but can also affect other parts of the body, such as the bones, kidneys, and lymph nodes. This study was conducted to implement care for patients with pulmonary tuberculosis, focusing on the problem of ineffective airway clearance. This is addressed through the implementation of chest physiotherapy and effective coughing techniques at the South Tangerang City Hospital. The method used was a case study involving the nursing process, which includes various stages: assessing the situation, determining the nursing diagnosis, planning interventions, implementing procedures, and evaluating the actions. Based on the results of the case study, three main diagnoses were found: Ineffective Airway Clearance, Nutritional Deficit, and Knowledge Deficit. The main nursing actions focused on managing Ineffective Airway Clearance by implementing chest physiotherapy and effective coughing techniques. The goal of the intervention was to help remove retained secretions using techniques such as postural drainage, percussion, chest vibration, and effective coughing exercises. This procedure lasted 15 to 30 minutes and was performed 1 to 3 times a day. After providing nursing care for 3 times 8 hours, the results showed an increase in the ability to cough effectively, decreased mucus production, reduced difficulty breathing, reduced additional breath sounds in the form of rhonchi, increased respiratory rate, increased appetite and frequency of eating, and increased patient understanding of pulmonary tuberculosis. Therefore, the application of chest physiotherapy and effective coughing is considered capable of overcoming the nursing problem of Ineffective Airway Clearance, which helps expel mucus. Thus, this scientific paper is expected to be a reference for health workers, hospital services, students, and educational institutions in providing nursing care to Pulmonary Tuberculosis patients with Ineffective Airway Clearance problems through the application of chest physiotherapy and effective coughing.
