Abstract
Ventilator-Associated Pneumonia (VAP) is one of the most common infections in intensive care units after patients undergo mechanical ventilation for 48 hours. Oral hygiene is one of the main strategies for preventing VAP. The use of natural-based mouthwash is now considered a potential alternative to chlorhexidine. This study aimed to analyze the effect of propolis extract mouthwash in oral hygiene management to prevent VAP in ICU patients using a holistic nursing case study approach. The instruments used included the Beck Oral Assessment Scale (BOAS) and the Modified Clinical Pulmonary Infection Score (MCPIS). After 7 days of intervention, the results showed that Patient P experienced a decrease in BOAS score from 16 to 9, while Patient S’s BOAS score decreased from 15 to 9. Meanwhile, the MCPIS showed a score of 4 and 3, which indicates that there was no development of VAP in both patients. Oral hygiene using propolis extract mouthwash can be a recommended alternative to support VAP prevention in ICU patients.
