Abstract
Patient safety is one of the key indicators of hospital service quality, and nurses’ speaking up behavior plays an important role in preventing incidents that may compromise patient safety. An initial assessment in the Inpatient Ward of RSUD Tarakan showed that nurses’ willingness to express concerns related to patient safety was still suboptimal. The pre-test results of the Speaking Up About Patient Safety Questionnaire (SUPS-Q) administered to 10 staff nurses showed mean scores of 7.6 for Perceived Concern, 10.0 for Speaking up, 10.8 for Psychological Safety for Speaking Up, and 6.8 for Encouraging Environment for Speaking Up, while Withholding Voice and Resignation towards Speaking Up were relatively high at 15.6 and 12.2, respectively. To improve this condition, a three-day mini-workshop on assertive communication was conducted (14–16 July 2025) using short lectures, interactive discussions, and role-play, involving 10 staff nurses. The effectiveness of the intervention was evaluated using pre–post SUPS-Q measurements. After the intervention, the mean scores of Perceived Concern, Speaking Up, Psychological Safety for Speaking Up, and Encouraging Environment for Speaking up increased to 11.0, 15.7, 19.6, and 12.5, respectively, while Withholding Voice decreased to 10.1 and Resignation towards Speaking Up decreased to 6.7 (p < 0.05). These findings indicate that the mini-workshop on assertive communication had a positive effect on improving nurses’ speaking up ability, strengthening psychological safety, and enhancing the perceived supportiveness of the work environment in the context of patient safety.
