Abstract
Dyspnea is one of the main problems and most common symptoms experienced by patients with acute decompensated heart failure. This condition arises from various disturbances in the respiratory process that cause discomfort or difficulty in breathing. The purpose of this paper is to describe nursing care for a patient with acute decompensated heart failure presenting with dyspnea through the application of mindfulness breathing training therapy to reduce the severity of shortness of breath in the Emergency Department of Pasar Minggu General Hospital. The method applied was a case study using the nursing process approach, along with the implementation of Evidence-Based Nursing (EBN). The assessment identified several nursing problems, including decreased cardiac output, hypervolemia, and anxiety. The interventions implemented consisted of acute cardiac care, fluid volume management, and relaxation therapy. The mindfulness breathing training therapy was carried out in a single 20-minute session, focusing attention on the breathing process. Dyspnea severity was evaluated using the Edmonton Symptom Assessment System (ESAS) dyspnea scale. The results showed a decrease in the average ESAS score, an increase in average oxygen saturation (SpO₂) to 99%, as well as improvement in blood pressure and respiratory rate within normal limits. All identified nursing problems showed improvement. In conclusion, mindfulness breathing training proved to be effective in reducing dyspnea among patients with acute decompensated heart failure. Therefore, this therapy is recommended as an alternative non-pharmacological intervention to help alleviate shortness of breath in patients with similar conditions.
