Abstract

Human Immunodeficiency Virus (HIV) remains a significant global health issue, including in Indonesia. Since 2022, Indonesia's antiretroviral therapy (ART) guidelines have recommended tenofovir–lamivudine–dolutegravir (TLD) as a first-line regimen. Although effective in suppressing viral replication, evidence regarding its renal effects remains limited and inconsistent. This study aimed to evaluate changes in kidney function after six months of TLD use, using urea, creatinine, and eGFR as parameters. A retrospective cross-sectional study was conducted using 50 medical records of HIV patients at RS PELNI Jakarta from 2022 to 2024. Bivariate analysis was performed using a paired T-test or, as an alternative, a Wilcoxon test. Univariate analysis described sociodemographic characteristics, showing that most patients were male, aged 25–49 years, had completed high school, were employed, unmarried, had homosexual-transmission risk, and were in clinical stage 1. Bivariate analysis (paired t-test or Wilcoxon) demonstrated significant increases in urea (p = 0.006) and creatinine (p < 0.001), and a significant decrease in eGFR (p < 0.001) after six months of therapy. This study found a significant change in kidney function among HIV patients receiving ART with the TLD regimen. Although still within normal ranges, these changes indicate the importance of routine kidney function monitoring among HIV patients receiving TLD.
