Abstract

Bell’s Palsy occurs due to damage to the facial nerve (nervus facialis), resulting in sudden weakness on one side of the face. This condition causes distinctive facial asymmetry and interferes with normal functions such as closing the eyes, speaking, eating, and drinking. The etiology of Bell’s Palsy is generally unknown or idiopathic. The aim of this case study is to evaluate the physiotherapy management of a Bell’s Palsy case. Methods: The data collection method in this case study involved one patient sample, Miss A, an 18 years old female, with a medical diagnosis of Bell’s Palsy Sinistra. She presented with complaints of pain and dysfunction in the facial muscles, including difficulty smiling, raising and furrowing the eyebrows, closing the left eye tightly, and drinking from a glass. The intervention was conducted over four sessions within a three-week period at Permata Pamulang Hospital. Results: After interventions using Electrical Stimulation (ES), Infrared, Massage, and Mirror Exercise, there was a reduction in resting pain from a scale of 3.2 and movement-related pain from a scale of 4.6 to 0, measured using the Visual Analog Scale. There was an increase in facial muscle strength of the m. frontalis and m. zygomaticus from grade 1 to grade 3 according to Daniel’s and Worthingham Manual Muscle Testing. Functional improvement in smiling movements increased from a total score of 38 to 56 using the Ugo Fisch scale. An improvement was also observed in the Facial Disability Index, with the physical function score increasing from 45 to 52. Conclusion: Physiotherapy interventions using Electrical Stimulation (ES), Infrared, Massage, and Mirror Exercise in a Bell’s Palsy patient demonstrated reduced pain, increased facial muscle strength, and improved functional ability of the facial muscles.
