Abstract
Chronic diseases like hypertension and type 2 diabetes are prevalent in Lebak Regency, affecting an estimated 20,405 individuals with hypertension. However, only 922 patients (4.52%) receive treatment. To address this, the Prolanis program evaluation aims to prevent and reduce complications. This study examines program implementation, supporting and hindering factors, and strategies to enhance Prolanis activities. Employing a qualitative case study approach, in-depth interviews, document observations, and field observations were conducted. Informants were purposively selected. The findings revealed suboptimal Prolanis activities due to the absence of home visits, SMS reminders, and RPPT achievements not meeting BPJS criteria. Supporting factors include Labkesda support and cadre collaboration, while hindering factors include double job of implementing officers, inadequate facilities, extensive puskesmas coverage, and the lack of a fixed Prolanis schedule. Strategies involve assigning clear roles to multiple chronic disease program leads, procuring routine facilities, enhancing cadre collaboration to record type 2 DM and hypertension cases, and establishing a fixed Prolanis schedule.
