Abstract

Nonconformity of Pharmaceutical Services and Drug Use (PKPO) with standards can increase the risk of medication errors, which can endanger patient safety. Medication error is one of the injury factors that can be anticipated in the health system. This research aims to evaluate the implementation of PKPO in preventing medication errors at Bhakti Kartini Hospital in 2023. This research uses qualitative methods with a case study approach. The results and discussion show that 6 of the 7 indicators of PKPO implementation have gone well. However, the storage indicators are still not up to standard, only reaching 75% (<80%). This is due to incomplete nutritional products and less than optimal staff for storing medicines. Thus, the overall implementation of PKPO in preventing medication errors at Bhakti Kartini Hospital only reached 90%. Other factors, such as staff non-compliance and inaccuracy, a lack of patient openness in the reconciliation process, and inadequate storage facilities, also hinder its implementation. The strategy to optimize this is that hospitals need to hold regular training, carry out regular monitoring and evaluation to ensure staff compliance and accuracy, provide outreach regarding medication errors, provide incentives, evaluate drug supplies, involve patients in reconciliation, and make improvements to storage facilities.
