Abstract

Ventilator-associated pneumonia (VAP) is a common and serious nosocomial infection among patients receiving mechanical ventilation in the Intensive Care Unit (ICU). Understanding the risk factors associated with VAP is essential for its prevention and effective management. This study aims to determine the relationship between the duration of mechanical ventilator use, patient age, and bacterial patterns with the incidence of VAP in the ICU of Gatot Soebroto Army Central Hospital from 2019 to 2023. This study is a quantitative research using an observational analytic method with a cross-sectional design. The sample selection was conducted using consecutive sampling at Gatot Soebroto Army Central Hospital (n=97). Data on the duration of ventilator use, patient age, and bacterial patterns were collected and analyzed (univariate and bivariate) and will be presented in table and narrative formats. The analysis results show a significant relationship between the duration of mechanical ventilator use and the incidence of VAP. Prolonged use of mechanical ventilators was found to increase the risk of VAP. Additionally, patient age was identified as a significant factor, with older patients having a higher risk of developing VAP. The bacterial patterns detected in patients also influenced the incidence of VAP. Gram-negative bacteria such as Acinetobacter baumannii and Klebsiella pneumoniae were predominantly found. There is a significant relationship between bacterial patterns and the incidence of VAP (p=0.011). The findings of this study highlight the importance of considering the duration of mechanical ventilator use, patient age, and bacterial patterns when assessing the risk of VAP in ICU settings. Implementing preventive measures, such as regular assessment of ventilator need and infection control protocols, can help reduce the incidence of VAP and improve patient outcomes. Further research is needed to explore additional factors and interventions that can contribute to the prevention and management of VAP.
