Abstract

Background: The aim of this research is to provide an overview of nursing care for Mr. R with problems with sensory perception disorders, auditory hallucinations at Dr. Mental Hospital. Soeharto Heerdjan. Research method: this research is descriptive research with a case study research design. Results: from this research, the assessment carried out found predisposing factors, precipitating factors, signs and symptoms, sources of coping, coping mechanisms. The predisposing factor found in the client was an unpleasant past experience, namely bullying experienced during college and the precipitating factor, namely not having a job for less than a year, which made the client lock himself up in his room. Three nursing diagnoses were successfully confirmed, namely sensory perception disorders, hallucinations, social isolation, risk of violent behavior. Interventions were successfully provided based on priority nursing diagnoses, which were first carried out for the problem of sensory disorders, perception of hallucinations, followed by problems of risk of violent behavior and social isolation. Based on the evaluation results given to Mr. R with a nursing diagnosis of sensory perception disorder hallucinations has provided an improvement in the client's condition. Nursing implementation carried out with hallucination management interventions, as well as appropriate implementation strategies, is able to reduce signs and symptoms of hallucinatory sensory perception disorders.
