Abstract
Background: Chronic Kidney Disease (CKD) is a condition that causes kidney damage by causing problems with the kidneys, so that toxins that should come out of the kidneys do not come out and cause damage to the kidney urethra, which causes urea levels to increase in the body and other kidney cells cannot function properly. Objective: The aim of this research is to collect direct information and provide nursing care which includes assessment, diagnosis, planning, implementation and evaluation of nursing for chronic kidney disease patients. Method: The research design used was a descriptive method in the form of a case study in implementing nursing care for chronic kidney disease on hemodialysis in the carnation room at RSU South Tangerang City. Results: The problems found in the cases are hypervolemia related to disruption of regular mechanisms, ineffective breathing patterns related to difficulty in breathing effort, instability of blood glucose levels related to insulin resistance, activity intolerance related to an imbalance between oxygen supply and demand. After carrying out nursing actions for 3x8 hours, the results of the evaluation of two diagnoses were resolved, two diagnoses were partially resolved. Conclusion: It is hoped that in the future researchers will be able to provide adequate and appropriate nursing care to patients with chronic kidney disease, tailored to the patient's problems.
