Abstract
Background: The purpose of this study is to provide an overview of nursing care for Mr.S with social isolation problems in Dr. Soeharto Heerdjan mental hospital. Research Method: The research method used in this research is descriptive research with a case study research design. Result: The results of this study are the assessment conducted found predisposing factors, precipitation factors, signs and symptoms, coping sources, coping mechanisms. Predisposing factors found in clients are the client's inability to find work and a partner, and the precipitating factor is parting with his family which causes the client to be reluctant to interact. Three diagnoses were successfully established, namely Social Isolation, Chronic Low Self Esteem, and Sensory Perception Disorder: Hallucinations. Based on the results of the nursing evaluation given to Mr. S, the client's condition has improved. S has provided an improvement in the client's condition. Nursing implementation is carried out with the promotion of socialization, promotion of self-esteem, hallucination management, and appropriate implementation strategies, so as to improve the client's socialization skills.
