Abstract

PT OPQ is a manufacturing company engaged in the energy sector, specifically "hardware and accessories for transmission and fiber optic lines" in the manufacture of distribution electrical components for electricity projects in Indonesia and abroad. Based on historical data from PT. OPQ in March 2021 –February 2022 has produced a total of 1358 pcs with a total of 165 defects or around 12.15%. This study aims to identify the factors that cause defects and how to minimize the number of defects and provide suggestions for improvements using lean six sigma and FMEA methods. The result of this research is the occurrence of defect appearance, dimension, assy/function and label/marking. The defect is caused by 3 human factors, machines and materials. Calculation of DPMO obtained a sigma value of 3.38 which means the company has not met the minimum threshold for the sigma value of 3.4 for company standards. Defect appearance is the highest defect or the most with a percentage of 47.27% and a RPN value of 1206 is given. Therefore, suggestions for improvement are given by holding training for operators in operating machines and carrying out supervision.
