Abstract

Background: Bell's Palsy is the most commonly found cranial nerve palsy VII and is generally idiopathic. This condition is characterized by weakness of the facial muscles which can cause facial asymmetry, speech and eating disorders, and emotional expression. In Indonesia, Bell's Palsy is one of the most common causes of acute peripheral facial nerve paralysis, with an incidence rate of about 10–30 cases per 100,000 population each year. Several factors such as diabetes, pregnancy, obesity, and hypertension are thought to play a role as risk factors for the occurrence of Bell's Palsy. Symptoms usually appear acutely within 24–72 hours and can be accompanied by pain around the ears, dry eyes, and impaired sensation in the tongue which has an impact on the patient's psychological condition. Physiotherapy plays a role in helping the recovery process through the provision of modalities and exercise therapy. Objective: To determine the management of physiotherapy in cases of Bell's Palsy. Methods: This study used a case study method that was carried out in March 2026 at RSKO with a 40-year-old male patient with facial complaints that it felt stiff and difficult to move on the left side, and experienced facial function disorders, namely difficulties when gargling, smiling and whistling. The physiotherapy examinations carried out were Manual Muscle Testing (MMT), Ugo Fisch Scale, and Facial Disability Index (FDI). Results: After four rounds of therapy using Infrared, Massage, and Mirror Exercise, there was an increase in facial muscle strength, namely in M. Zygomaticus from a value of 3 to 5 M. Procerus from a value of 1 to 3. The Ugo Fisch Scale has increased from 78 to 87. In addition, patients show an improvement in facial functional abilities in daily activities. Conclusion: After four sessions, infrared therapy helped reduce tension and stiffness in the facial muscles. Massage provided a relaxing effect and improved muscle strength and function, while mirror exercises helped improve movement control and muscle strength through visual stimulation that supported the recovery process.
