Abstract
Background : Osteoarthritis genu is a degenerative disease of the knee joint that often occurs in the elderly due to the aging process, especially in the musculoskeletal system. This condition is characterized by damage to joint cartilage that can cause pain, stiffness, limited range of motion of the joint, osteophyte formation, and decreased ability to perform daily functional activities. In Indonesia, osteoarthritis genu is generally experienced by individuals aged over 50 years and can develop progressively if not treated properly. Objective : This study aims to determine the results of physiotherapy implementation in reducing pain using Transcutaneous Electrical Nerve Stimulation (TENS), Ultrasound, Isometric Exercise, and Heel Slide modalities in cases of bilateral osteoarthritis genu. Research Method : This study used a case study method on a 59-year-old female patient with a diagnosis of bilateral osteoarthritis genu at RSPAD Gatot Soebroto. The patient's main complaints were pain, stiffness, and decreased functional activities. The examination instruments used included the Visual Analog Scale (VAS) to measure pain, Manual Muscle Testing (MMT) for muscle strength, a goniometer for range of motion (LGS), and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) for functional ability. Results : After the provision of physiotherapy intervention, there was a decrease in silent pain by 1.65 points, tenderness by 2.7 points, and pain on movement by 2.75 points. An increase in LGS occurred in bilateral knee flexion movements, namely right knee flexion increased by 15˚ (AROM) and 10˚ (PROM), while left knee flexion increased by 20˚ (AROM) and 15˚ (PROM). Left hamstring and quadriceps muscle strength increased to 5/5, while the right remained at 5/5. The WOMAC score also decreased to 33, including the moderate dependence category. Conclusion : Providing Transcutaneous Electrical Nerve Stimulation (TENS), Ultrasound, Isometric Exercise, and Heel Slide interventions in cases of bilateral genu osteoarthritis can help reduce pain.
