Abstract
Type 2 diabetes mellitus (T2DM) is a chronic metabolic disease that is frequently accompanied by metabolic syndrome, although a small proportion of patients present with a lean type phenotype characterised by a normal or underweight body mass index. Metformin is the first-line therapy, given either as monotherapy or as dual therapy with glimepiride, and has been reported to influence haematological parameters through inflammatory and oxidative stress pathways. This study aimed to analyse differences in haematological parameters, namely haemoglobin, haematocrit, mean corpuscular volume (MCV), mean corpuscular haemoglobin (MCH), mean corpuscular haemoglobin concentration (MCHC), leukocyte count, and platelet count, after metformin monotherapy and dual therapy in lean type T2DM patients and in T2DM patients with metabolic syndrome. This research adopts an observational analytic approach with a retrospective cross-sectional design using medical record data of 148 patients at Persahabatan General Hospital from January 2023 to January 2026, comprising 62 lean type T2DM patients and 86 T2DM patients with metabolic syndrome who had received therapy for at least six months. Data were analysed univariately and bivariately using the Chi-Square or Fisher Exact test. In the lean type T2DM group, no significant differences were found in any haematological parameter between monotherapy and dual therapy (p>0.05). In the T2DM with metabolic syndrome group, no significant differences were found in any haematological parameter between the two therapies either (p>0.05). It can therefore be concluded that the type of metformin-based therapy does not significantly affect the haematological profile in each group within six months of observation.
