Abstract

Coronary Heart Disease (CHD) is one of the leading causes of death in Indonesia and requires long-term combination therapy, thus increasing the risk of drug interactions, especially in patients with comorbid type 2 diabetes mellitus. This study aims to analyze the relationship of potential drug interactions with outcomes of blood pressure, fasting blood sugar (FBS), and HbA1C in CHD patients with type 2 DM at Fatmawati General Hospital in 2024–2025 using a cross-sectional design. A total of 96 patients were analyzed, predominantly female (62.5%) with the highest age being 55–64 years (47.9%). In the CHD Therapy Regimen, patients received several groups of drugs, the most commonly used antiplatelet drug was clopidogrel, the most commonly used antihypertensive drug was bisoprolol, and the most commonly used antihyperlipidemic drug was atorvastatin. 505 potential drug interaction events were found, with pharmacodynamic mechanisms as the dominant mechanism for blood pressure (93.3%) and glycemic levels (95.0%). Most potential drug interactions were moderate in severity, affecting both blood pressure (68.3%) and GDP and HbA1C (88.4%). Statistical analysis showed a significant association between potential drug interactions and outcomes of blood pressure (p = 0.003), GDP (p = 0.016), and HbA1C (p = 0.023). However, the analysis did not specifically examine the relationship between the mechanism and severity of potential drug interactions and therapeutic outcomes.
