Abstract
During menstruation, many adolescent girls experience lower abdominal pain known as dysmenorrhea. In Indonesia, the prevalence of dysmenorrhea among adolescent girls reaches 64.5%, particularly in urban areas such as West Java. The high prevalence of dysmenorrhea is a concern because it not only causes physical discomfort but can also interfere with daily activities, concentration, and learning processes among adolescents. Various factors are thought to influence hormonal balance that may contribute to the occurrence of dysmenorrhea, including stress, physical activity, and nutritional status. Therefore, this study aimed to determine the association between stress, physical activity, and nutritional status and the occurrence of dysmenorrhea among adolescent girls aged 15–18 years at SMAN 9 Depok. This study employed a cross-sectional design involving 73 participants. Data were collected through questionnaires, while nutritional status was assessed directly using anthropometric measurements and plotted using the Centers for Disease Control and Prevention (CDC) growth charts. The results showed that 13 students experienced stress (17.8%), 20 students had low physical activity levels (17.8%), 38 students were classified as having malnutrition (52.1%), and 39 students experienced dysmenorrhea (53.4%). Dysmenorrhea was not significantly associated with stress based on the chi-square analysis (p = 0.117). In contrast, physical activity (p = 0.020) and malnutrition (p < 0.001) were significantly associated with the occurrence of dysmenorrhea. Multivariate analysis of the three variables showed that nutritional status was the most influential risk factor for dysmenorrhea. Students with malnutrition had a 6.424-fold higher risk of experiencing dysmenorrhea. Although physical activity and nutritional status were significantly associated with dysmenorrhea, nutritional status had the strongest influence. Meanwhile, stress was not significantly associated with the occurrence of dysmenorrhea in this study.
